FACIAL CLUB MEMBERSHIP AGREEMENT
A credit card will be held on file and charged $125 on the first of the month following the date of this contract.
The $125 will be stored in your Facial Club funds vault to be used towards services with the remaining balance charged at check-out.
Products are purchased separately at a 15% discount.
Members receive 15% off of products and services; and 10% off of gift cards for the active months of membership.
Membership is for a minimum 6-month period.
After the initial membership period you can cancel at any time with a 7 day notice. If you cancel, membership cannot be reactivated for 1 year.
Member must abide by cancellation policy. No-shows or late cancellations within 24 hours will forfeit your service for that month without a refund.
Membership benefits available with contract only.
If you miss a month of services you forfeit services for that month. No refunds will be given for any remaining unused sessions.
Membership discounts cannot be combined with any other specials or offers and are not to be used for a Unicorn Treatment series. You choose between the membership discount or special offer.
Service Upgrades: your monthly credit will be applied towards your upgraded services and you pay the difference (less your discount) at the time of checkout.
Members who cancel for any reason prior to the 6-month minimum commitment will be charged back the discount amounts for all products and service received using the credit card on file. An itemized invoice will be provided.
By submitting this form you certify that you understand and agree to the terms and conditions as outlined above and that your credit card on file will be charged $125 on the first of the month following the date this agreement was signed.
